5 Why Analysis Add-on

The 5 Why Analysis add-on offers incident investigators an additional tool in assessing the root cause of a reported incident or near miss. The add-on will dynamically generate new "why?" questions to help users identify underlying causes. Once the root cause has been found, marking it as such in the add-on will write the results into the Report field on the Investigation tab automatically.

The add-on functionality is available by default in the Incident Starter Pack as of version 475. We packaged this as an "add-on", since this allows us to introduce the component into older installations as well. This requires some manual work on TenForce' side.

Details

• Component can be enabled by TenForce;
• Component will generate a new "Why did this happen?" questions after clicking "Why?";
• Each answer can be marked as a root cause;
  • Marking an answer as a root cause will make the component read-only – it is no longer editable;
  • Marking an answer as a root cause will automatically copy the data into the Report field;
    • All added data in the Report field is appended – no information is overwritten;
• The first why question answered will generate a new "Why did this happen?" question and will cause the "Start over" button to appear below the element;
  • Clicking "Start over" will clear the data in the 5 Why component and will restart the process at the first "Why did this happen?";
  • The "Start over" button is also available when a root cause has been marked;
